BMO offers medical coverage through the Consumer Choice Plan, a high-deductible health plan with a tax-advantaged Health Savings Account, dental coverage through Delta Dental, and two vision plan options through VSP.

If you don’t enroll within 31 calendar days from your hire date, your next opportunity to enroll will be during Annual Enrollment, unless you experience a qualifying life event.

Below is an overview of the health care options for 2024.

Medical Overview
Consumer Choice Plan
  • The Consumer Choice Plan is a High-Deductible Health Plan (HDHP) that includes a Health Savings Account (HSA). The plan is administered by Blue Cross Blue Shield of Illinois (BCBSIL) and Kaiser (Colorado, California, Oregon).
  • The BCBSIL Consumer Choice Plan includes prescription drug coverage through Express Scripts. Review the 2024 Express Scripts National Formulary.
  • The Kaiser Consumer Choice Plan includes prescription drug coverage through Kaiser. You can view regional formularies below:

How the Plan Works
  • In-network preventive care is covered 100%, no deductible.
  • You pay the full cost of other services, including prescription drugs, until you meet the annual deductible. Then you pay coinsurance and the plan pays the balance.
  • Once you meet the annual out-of-pocket maximum, the plan pays 100% of covered expenses for the rest of the year.
  • You can use any provider you want, but you’ll pay less when you use in-network providers. Depending on where you live, you may have more than one provider network to choose from.
  • The Consumer Choice Plan includes an HSA to help you get the most of your health care dollars. An HSA is a tax-free account you can use to pay eligible health care expenses now or in the future.
  • Medical plan rates will be based on a five-tier total compensation structure.

What You Pay
  • In 2024, there will be 5 tiers of medical rates based on total compensation.
  • Total compensation includes your base salary, overtime, shift differential and variable pay you earned between October 1, 2022 and September 30, 2023.

Dental Overview
Delta Dental Plan You’ll maximize your benefits by receiving care from a Delta Dental PPO or Delta Dental Premier network dentist:
  • Delta Dental PPO: Lowest out-of-pocket costs and network protection.
  • Delta Dental Premier: Higher out-of-pocket costs than PPO, but may be lower than non-network and network protection.
  • Non-network: You may have the highest out-of-pocket costs.
  • $2,000 annual maximum benefit per person.
Plan Features
  • Annual deductible: $50 for individual coverage; $150 for family coverage.
  • Preventive services: Covered 100%, you pay nothing.
  • Basic services: You pay 20%.
  • Major services: You pay 50%.
  • Orthodontic care: You pay 50% up to a $2,000 orthodontic lifetime maximum per person.
Vision Overview
VSP Vision Options You have two vision plan options – Base Plan and Premier Plan. With both plans, you can see any vision care provider, but you’ll get the most out of your benefits if you use a network doctor.

  • WellVision exam every calendar year: $10 copy
  • Retinal screening: $20 copay
  • Additional exams as needed: $20 copay
  • Lenses every calendar year (single, lined bifocal, lined trifocal lenses: $20 copay then covered in full
  • Frames every other calendar year: $150 allowance for retail frames
  • WellVision exam every calendar year: $0 copy
  • Retinal screening: $20 copay
  • Additional exams as needed: $20 copay
  • Lenses every calendar year (single, lined bifocal, lined trifocal lenses: Covered in full
  • Frames every other calendar year: $175 allowance for retail frames
